About This Course

This course is designed for personnel assigned to conduct and manage major accident or incident investigations, as well as those designated to perform such investigations. Participants are taught to focus their activities on defining the facts and circumstances related to the event, determining its causes, and developing remedial actions to control associated risks.

Participants also learn to use nine guidelines for effective interviewing and practice conducting interviews. They become skilled at identifying different types of evidence, including paper evidence contained in various records, and applying a loss causation/problem-solving model to analyze the evidence. The course includes participation in mock accident and incident investigations, including completing the appropriate information in an accident investigation report.
